Register for Online Services
To register for online services please complete this form. You then have two choices for your ID check.
- You can either use the NHS app to access your records, which will undertake the ID checks using your phone or tablet.
- You can use Patient Access online. You will need to bring in photo ID such as passport or photo driving licence so we can undertake the ID check and we will then text you an access code.
You can also visit the practice, bringing with you two forms of identification. One of these items should include your photograph.
We will then issue you a username and password.
Once you are registered you will be able to use the service to:
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- View your medical record
We currently have 2 options for Online Access for Strawberry Hill Medical Centre patients:
Patient Access
This is a website which can be accessed on a computer/laptop rather than an app to be downloaded. This is for any patients who don’t have access to a smart device.

NHS App
This is an app which can be downloaded on Apple and Android devices. Patient are asked to set up a new NHS login by verifying your own ID by taking a photograph of your photo ID and then recording a short video of you saying 3 or 4 numbers in order for the system to recognise you are the person on your ID. If you are not able to do this then you can come into the surgery to get a linkage key, ODS code and account ID from reception. This app is for over 13s only.

Access To Medical Records
Newly registered patients are now entitled to access a summary of your medical record from the point of registration onwards and therefore we will need to see two forms of ID at registration. If you would like to have access to your full medical record (for online services only), please complete our online form.
